NHS Tameside and Glossop Provider Division now offers three diabetes education programmes.
The DAFNE Programme
The DAFNE programme is for patients with Type 1 diabetes. Patients can be referred via their GP or Practise Nurse; self-referral is not accepted. Diabetes Specialist Nurse assessment will be
performed prior to being listed for the course, to ensure insulin therapy and blood glucose control are appropriate. Any changes to treatment will be made at this point.
Courses will run Monday to Friday 9am – 5pm with a ½ day follow-up session after 6 weeks and annual assessment thereafter. The programme is delivered from a variety of locations across Tameside and Glossop.
The course is fully funded, at no cost to the attendee. No pre-course work is required, but attendees need to ensure that they bring their insulin and blood glucose monitoring equipment. A good supply of test strips is required as testing will be performed at least 4 times a day for the duration of the course.
The X-PERT Programme
The X-PERT programme is for patients with Type 2 diabetes, diagnosed within the last year. Patients can be referred via GP or Practise Nurse; self-referral is not accepted. Courses run for ½ day a week over 6 weeks, all sessions must be attended and refreshments will be provided. The course is fully funded, at no cost to the attendee, no pre-course work is necessary. The programme is delivered from a variety of locations across Tameside and Glossop.
Living with Diabetes Sessions
Living with Diabetes is a group session for any patient who has had diabetes for more than 12 months and / or their carers who want to know more about diabetes. Living with Diabetes consists of a single 4 hour session with lunch and refreshments provided. The course is fully funded, with no cost to the attendee. Patients are able to bring a friend or relative and can send a carer or relative on their behalf, if they are unable to attend. Although this is a single session, attendees can return in the future if they require updating. Sessions are held from 10am – 2pm, at different locations across Tameside and Glossop.
Health Improvment Service
Diabetes Education Service has close links to the Health Improvement Service, which is available in the community to help and support people in making positive lifestyle changes. The programmes available include: Stopping Smoking, Weight Management, One-to-One Lifestyle Coaching and can signpost to appropriate physical activities including gentle sessions for people with health issues.
